Gaps in treatment: measuring them from the records
A gap in treatment is a stretch of time between visits. Both sides of a claim read it, so it should be measured, dated and cited.
Measure it from the pages
A gap runs from the last visit before it to the first visit after it. Count the days between the dates printed on those two pages.
Use the date of service. A note dictated or signed days later still records the date of the visit.
Two kinds of gap
- A gap in care. No visit happened. The next note may say why: a scheduling delay, a planned wait for surgery.
- A gap in the file. Visits happened and their records did not arrive. A referral, a bill or a later note often names the missing visit.
The second kind calls for more records. It says nothing about the patient’s care.
The threshold is the reader’s
Firms and carriers each set the length of gap worth flagging. A table that lists every interval lets each reader apply their own.
What Claracord delivers
Each case includes a table of the gaps between visits. It is arithmetic on the dates printed on the pages, and it states no cause.
The provider and custodian index lists the records your set mentions but does not contain.
Human-verified. A complete sample case is on the home page. Synthetic demo case.
