A medical records index: what each entry needs
An index is the table of contents of a record set. A good one says where a document is before anyone opens the file.
One row per document
- Date of service, as the document prints it.
- Provider and facility.
- Document type: office note, operative report, imaging, bill.
- Page range, by Bates number.
Each row links to its first page. A link to the whole file sends the reader looking.
By date, by provider, or both
A date-ordered index shows the course of care. A provider-ordered index shows what each custodian sent. Most files need both views of the same rows.
The records the set does not contain
Records name other records: a referral, an MRI ordered, an outside lab. An index that lists those mentions shows what the file is missing.
Index or bookmarks
Bookmarks put the index inside the PDF, one per document. A separate index can also be filed, served and cited.
What Claracord delivers
The record set comes with a page index: what is in the file, and where. The provider and custodian index lists the records your set mentions but does not contain.
