A medical billing ledger: one line per charge
Medical specials are a sum of charges. Each charge in the sum should lead to the bill that states it.
What each line carries
- Provider.
- Date of service.
- Description or code, as the bill prints it.
- Amount charged.
- The page of the bill.
Charged, paid and owed
A bill states a charge. Payments and adjustments come from other documents: explanations of benefits, remittances, lien statements.
A ledger built from the medical records reports charges. Paid and owed amounts need the payment records too.
Three checks worth running
- Every provider in the chronology has a bill, or a note that none arrived.
- A charge appears once, even when its bill was produced twice.
- The total is the sum of the lines shown.
