Medical summary or medical chronology: the difference
The two names get used for one product. They are different documents with different jobs.
A medical summary
A summary is a narrative, usually grouped by injury or by provider. It selects and explains, and it reads well in a demand or a claim report.
Its value rests on the writer’s judgment. A reader checks it by going back to the records.
A medical chronology
A chronology lists the encounters in date order. Each entry carries the date, the provider, what the page records, and the page.
It selects less and says less. A reader checks any entry in one step, at the page it cites.
Which one a file needs
- Discovery, depositions, expert preparation: the chronology. The questions there turn on what a page says and when.
- A demand or a claim report: a summary, written by the professional who owns the file.
- Both: the chronology first. A summary written from a cited chronology can carry its citations.
